HomeShare Vermont Helps people stay in their homes by connecting them with potential housemates who are looking for a place to live. The primary goal is to help elders stay at home. There are no age, ability or income restrictions to use their services. The services are tailored to the individual. HomeShare Vermont can help you find the right housemate or find a place to live or hire live-in or hourly care.
Special Services Transportation Agency SSTA provides coordinated transportation service to many area human service agencies. A few examples are the Visiting Nurse Association’s Adult Day Programs, Champlain Senior Center (an important meal site for many Burlington Elders), Howard Community Health Services, and Age Well. The transportation SSTA provides allows clients an opportunity for social interaction, job training and employment, medical services and therapy.
Meals on Wheels Meals On Wheels are delivered by community volunteers five days a week in the majority of the communities we serve. A hot nutritious meal is provided that includes protein, vegetables, milk, juice, bread, and fruit. Favorite dishes are served regularly and special diets are also available.
Green Mountain Transit Free transportation for Medicaid recipients and contracted ADA paratransit service for those who are unable to ride the bus.
Vermont Assistive Technology Program Assistive Technology is any piece of equipment, or product system, whether acquired commercially off the shelf, modified, or customized, that is used to increase, maintain, or improve functional capabilities of individuals with disabilities. The program offers, Device Demonstration, Device Loans, and Information & Assistance.
